Please FILL OUT and SUBMIT the intent form(pink form) to your teacher by October 17th . 

• Students need to submit a separate form for each teacher.
• Make sure the forms are completed with accurate information. 
• Use the Dual Credit Course Offering List in the above (link) to select the correct College Course Name on your online application. 
• Do not email a pic to your teacher. It's likely to get lost. However, It's best that you keep a pic of the signed form.
