• Students need to submit a separate form for each teacher.
• Make sure the forms are completed with accurate information. 
• Use the Dual Credit Course Offering List in the above (link) to select the correct College Course Name on your online application. 
• It's best that you keep a pic of the signed form. Do not email the pic to your teacher.
